In many cultures the dead live in a parallel world, affecting the living. Being present at the death, and hearing last words is important in all cultures. The final moments have a profoundly spiritual as well as emotional aspect.

HEALTH INEQUALITIES AND PALLIATIVE CARE There is considerable geographical inequality in provision for palliative care. The Palliative Care Survey 1999 (NCHSPCS 2000) showed 82% more palliative care in-patient beds in the North West and South East Regions than in Trent, 131% more day centre places in Trent than in London, nearly 100% more home care nurses per million of the population in the South West than in Trent. The ratio of doctors’ sessions per million of the population in home care was more than 400% greater in London than in the North West.
