23 Psychiatric Assisted Suicide Diane Pretty’s case clearly illustrates how much more at home we are with the concept of physical than of mental illness. If it were possible to show a defect or irreversible degeneration of the brain in every case of mental illness, then we might be able to deem some psychiatric patients to be rational in their desire to commit, or be helped to commit, suicide before things got worse for them. Their situation would be like that of Diane Pretty. But so far this is not possible.

She was severely brain-damaged, and it was believed, even by her parents, that she was unlikely to live for more than a few months. However, they prayed for a miracle and asserted that she ‘was not ready to die’, though she did not respond to stimulation and seemed to experience nothing but pain. She stopped breathing several times and each time was given ventilation to resuscitate her. Later in 2004 the child’s parents went to court in an attempt to overrule the doctors’ decision that, if their baby should stop breathing again, she should not be reventilated.

These people are not just ‘willing’ or ‘consenting’ to die; they are begging to be allowed to die; ‘willing themselves’ to die, perhaps, but this rather childish usage is not what ‘voluntary’ means. In fact, the word ‘voluntary’ ought perhaps to be dropped from the discussion. We should instead speak of ‘asked for’ or ‘requested’ or ‘chosen’ death, cumbersome though this may be, and contrast this with a death that is deliberately brought about but without the patient’s request. Diane Pretty’s case is not unique.
