Although carer stress and quality of life are key issues in the long-term management of disabled patients, these are often not given the attention they deserve in health outcome research. Factors influencing caregiver quality of life may be different from those influencing quality of life in patients. For example, performance as a caregiver, satisfaction with care received by the patients, and adjustments to the caregiver’s personal life are specific to carers’ perceptions of self-worth. 14 Most relatives who provide support have little respite: only a minority leave the patient unattended for all or part of the day.

However, children’s experience of ill health, medical intervention, and the effects of these upon development and adjustment are not well understood. It is assumed that hospitalisation, intrusive medical procedures, the uncertainty of survival, and prospects of disability have a negative effect upon children. In order to understand these effects and improve paediatric healthcare it is important that we are able to assess children’s quality of life effectively. A lack of conceptual models of health and quality of life in children has made it difficult to develop practicable, valid measures of child quality of life.

Few directly asked patients about those factors that constituted quality of life. Where they did involve patients, they asked about the impact of illness on people’s lives or behaviours (for example, the Nottingham Health Profile15 and the Sickness Impact Profile16) but not about the important things in life. The result is that there is a danger that most of the widely used measures do not address what is important to patients in determining their quality of life.
