Technology that is not central to the control of the train has also been implicated in serious rail incidents. For example, near Chatsworth in California (USA) two trains collided, leading to 25 fatalities and 135 injuries because one of the drivers missed a signal showing a red aspect, most likely because he was using a mobile phone (National Transportation Safety Board, 2010). In situations such as those outlined above, it is usually assumed that the driver is at fault for neglecting to properly assess the situation or for engaging in inappropriate actions.

In particular the approaches taken in the book show the limitations of a narrow technical perspective on new technologies. Most importantly the book illustrates the centrality of non-technical factors and the importance of participative design principles being applied both before and after the implementation of a new technology. At a practical level it shows the benefits of being able to make fast mistakes and provides some useful ways to ensure most of the mistakes can be identified and the operation made much faster and cheaper than has been the case when the issue is not addressed.
